杨崇祖，仇池氐人，在496年正月被南齐授为沙州刺史，封阴平王。
他在父亲杨炅去世后继立，死后由儿子杨孟孙继承。从《梁书》记载天监初年以“孟孙为假节、督沙州刺史、阴平王”推算，
杨崇祖在天监初年或以前去世。
