D R I V E R
Permanent Part-time, guaranteed 15 hours per week
between core hours of 10am to 2pm, Monday to Friday

Requirements:

 Provide safe transportation of clients within York Region and surrounding area
 Hold valid Ontario Driver’s License, with satisfactory driving record
 Must have reliable vehicle capable of carrying 3 people (or 2 plus a walker)
 Must be over 25 years of age per insurance requirements
 Excellent communication, customer service and organizational skills
 Understand and use excellent transferring techniques with clients, an asset
 Experience in a community setting working with the frail elderly, an asset




Requirements:

 Home At Last PSWs assist seniors to settle at home upon hospital discharge.
 Work between 5 to 10 hours per day, with guaranteed minimum of 5 hours/day
 Must have valid Ontario Driver’s License and reliable vehicle
 Must be willing to travel throughout York Region and surrounding area
 Rate: $14.15/hour plus travel time at regular hourly rate plus mileage

TRANSPORTATION SUPERVISOR
Permanent Full-time – 35 hours per week

Requirements:

 Lead development and coordination of CHATS transportation program for seniors throughout York Region and surrounding area, including supervision of drivers.
 Have relative education with 2 years experience working with seniors in the community, or equivalent combination of training and experience
 Demonstrated achievement in community relationship management and partnership development
 Strong scheduling experience
 Supervisory/ project management experience
 Excellent communication and problem-solving skills
 Leadership and coaching skills
